OPERATIONAL GUIDELINES

For unregistered drugs, you must receive the declaration of responsibility completed in all missing parts, stamped and signed by both the prescribing physician and the pharmacy department.

We will take care of sending the documentation to the USMAF offices for the release of the health clearance.

For drug shortages, the healthcare facility must send the completed documentation, with the missing sections filled in, stamped, and signed as required, directly to AIFA at the email address provided in the form.

AIFA will issue the authorization directly, which must then be subsequently sent along with the order.
